Position Overview


We are seeking a compassionate, motivated, and clinically skilled Nurse Practitioner (NP) to join our growing primary care practice on a full-time or part-time basis. The ideal candidate will provide comprehensive outpatient care to adult and/or pediatric patients while collaborating with physicians and clinical staff to deliver high-quality, patient-centered care.


This position is ideal for a provider who is comfortable working in a fast-paced environment and values preventative medicine, chronic disease management, and continuity of care.


Responsibilities

‍️ Clinical Duties


  •  Perform comprehensive patient evaluations, physical examinations, and medical histories 
  •  Diagnose and treat acute and chronic medical conditions 
  •  Develop and implement treatment plans 
  •  Order, interpret, and review laboratory and diagnostic testing 
  •  Prescribe medications and therapies in accordance with New York State regulations 
  •  Provide preventative care, wellness visits, and patient education 
  •  Manage chronic conditions including: 
    • Hypertension
    • Type 2 Diabetes
    • Obesity
    • Hyperlipidemia
  •  Perform documentation accurately and efficiently within EMR/EHR systems 
Administrative & Team Collaboration


  •  Collaborate with physicians, medical assistants, and administrative staff to ensure smooth patient flow 
  •  Maintain timely chart completion and documentation compliance 
  •  Participate in quality improvement and patient satisfaction initiatives 
  •  Assist with medication prior authorizations and care coordination when needed 

Qualifications

Education & Licensure


  •  Master’s or Doctorate degree from an accredited Nurse Practitioner program 
  •  Active New York State Nurse Practitioner license 
  •  Board certification as: 
    •  Family Nurse Practitioner (FNP), or 
    •  Adult-Gerontology Primary Care NP (AGPCNP) 
  •  Active DEA registration (or eligibility) 
Experience


  •  Minimum 1 year Primary care experience preferred (new graduates may be considered depending on training and background) 
  •  Experience with outpatient EMR/EHR systems 
  •  Strong communication and patient education skills 
  •  Bilingual candidates encouraged to apply (Spanish, Portuguese, etc.)

Schedule Options

Full-Time


  •  4–5 days per week
