Generation Clinical Partners (GCP) is seeking an experienced (minimum 2 years) Nurse Practitioner or Physician Assistant to join our medical practice.
The NP/PA serves as a critical member of the care team and will provide direct patient care, coordinate care transitions, and help lead quality improvement efforts in the senior living facility setting. The focus of this position is to provide comprehensive high quality, efficient, and responsive onsite care tailored to the unique needs of each resident.

Job Responsibilities
1. Within scope of practice, effectively manage acute and chronic conditions by developing and overseeing high quality, cost‐effective treatment plans for patients in the facility, including ordering and interpreting diagnostic tests and prescribing medications and other treatments as appropriate.
2. Provide patient/family education and counseling in the areas of self‐care, health promotion, maintenance, and disease prevention.
3. Work closely with collaborating physicians and other colleagues, which allows for consultation, collaboration, and/or referral.
4. Provide clinical leadership and education to nursing and other staff, including to facility performance improvement efforts.
5. Document all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals and consultations according to established standards in GCP's provider‐friendly electronic health record.
6. Analyze new knowledge gained from conferences, workshops, professional literature, or hands‐on training and assimilates this knowledge into clinical practice.
7. Maintain strict confidentiality with regard to protected health information and understands and adheres to GCP's HIPAA Privacy and Security policy.
8. Share in taking calls with other GCP providers.
Qualifications and Skills
1. Current valid and unrestricted RN and NP/PA licensure in Missouri (and/or Illinois)
2. National Board Certification and MSN in related fields are strongly preferred.
3. 2+ years experience, preferably in acute‐care, post acute care, or inpatient rehabilitation practice settings.
4. Demonstrated interpersonal skills to be approachable and understandable to patients, families, and staff.
5. Ability to be flexible to meet the practice schedule so as to ensure appropriate coverage and high quality patient care.
6. Collaborative practice style and ability to work harmoniously with others.
7. Demonstrated skill in resolving conflicts and promoting positive outcomes for patients
