Impact to Health has partnered with a mission-driven, privately owned primary care group in Birmingham, AL to find a Family Medicine or Internal Medicine physician ready to reclaim their clinical autonomy. In this physician-led, value-based organization, patient outcomes—not daily volume metrics—dictate your day.

Core Responsibilities:
Comprehensive Clinical Care: Examine patients of all backgrounds, diagnose conditions, order/interpret diagnostic tests (labs, imaging), and develop and implement personalized care plans for long-term conditions (e.g., diabetes, hypertension) and conduct wellness exams.
Team-Based Care & Coordination: Collaborate with internal and community providers, and specialists to manage care transitions, ensuring continuity, particularly for medically complex patients. Supervise and support advanced practice professionals.
Patient Advocacy & Communication: Build trusting, long-term relationships through effective, empathetic communication. Educate patients and families on disease management and healthy lifestyle changes.
Efficient Workflow & Technology Adoption: Utilize Electronic Health Record (EHR) systems to maintain accurate, up-to-date, and detailed documentation in the electronic medical record system.

Patient Base: Senior/ Adult Focused
Schedule: Monday–Friday
Care Team: Comprehensive support (MAs, Care Coordinators, Admin)
EMR: Athena
Core Focus: Preventive care, chronic disease management, and long-term patient continuity
Requirements
Qualifications:
Education: Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited medical school.
Certification: Board Certified/Eligible in Family Medicine, Internal Medicine, or a related field.
Licensure: Current unrestricted state medical license and DEA license.
Interpersonal Skills: Exceptional empathy, compassion, and the ability to build rapport with diverse, often vulnerable, patient populations.

Compensation estimates comprised of base salary & bonuses $300k-$400k.
