Primary Care Physician<\/span><\/b>

We are seeking a dedicated, patient -focused Primary Care Physician to
join a growing, value -based care practice serving a predominantly geriatric
population. This is a full -time, 100% outpatient opportunity offering a
collaborative, team -based environment with a strong emphasis on preventive
care, chronic disease management, and improving quality outcomes for seniors.<\/span>

Advantage patients in a model that prioritizes access, coordination, and

Key Responsibilities<\/span><\/span><\/b>
<\/p>- Provide comprehensive primary
care to a geriatric patient population with multiple chronic conditions<\/span>
<\/li> - Deliver patient -centered care
with an emphasis on preventive services and care coordination<\/span>
<\/li> - Develop and manage individualized
care plans in collaboration with patients, families, and interdisciplinary
teams<\/span>
<\/li> - Utilize virtual care tools to
monitor and manage high -risk patients between visits<\/span>
<\/li> - Ensure accurate and compliant
documentation, including risk adjustment and quality metrics coding<\/span>
<\/li> - Communicate effectively with
patients, caregivers, and other providers across the continuum of care<\/span>
<\/li> - Support improved clinical
outcomes and patient satisfaction through proactive engagement<\/span>

Qualifications<\/span><\/span><\/b>
<\/p>- MD or DO from an accredited
medical school (Family Medicine, Internal Medicine, or Geriatrics
preferred)<\/span>
<\/li> - Board Certified or Board Eligible
strongly preferred<\/span>
<\/li> - Active and unrestricted Florida
Medical License<\/span>
<\/li> - DEA license<\/span>
<\/li> - Current CPR certification (BLS
required; ACLS/PALS preferred)<\/span>
<\/li> - Experience managing geriatric
patients and Medicare Advantage populations required<\/span>
<\/li> - Familiarity with value -based care
models, risk adjustment (MRA), and quality measures strongly preferred<\/span>
