We are seeking a compassionate, collaborative clinician who thrives in a team-based environment and is deeply committed to patient-centered primary care.
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
Completed residency in Family Medicine or Internal Medicine
Board Certified or Board Eligible (FM or IM)
Active, unrestricted Florida medical license (or ability to obtain licensure prior to start)
A strong interest or prior experience in value-based, care models, or population health
A background working with Medicare or senior-focused populations
A passion for working within an interdisciplinary care team to manage complex chronic diseases
Familiarity with Athena or similar EMR systems
