This role presents a unique chance to provide exceptional, patient-focused care while contributing to the evolution of healthcare in our community.
Practice Highlights & Innovations
- Team-Based, Integrated Care:
- RN Care Managers: Assist all providers in managing risk-stratified patient populations and ensuring coordinated care for those most in need.
- Embedded Behavioral Health: Access to onsite Social Workers, Licensed Clinical Workers, Psychiatrists, and Advanced Practice Nurses for comprehensive behavioral health support.
- Collaborative Hospitalist Support: Smooth transitions and assistance for patients requiring inpatient care.
- Patient-Centered & Accessible:
- In-Person and Virtual Care: Provide patients with flexible options, including both onsite and full-time virtual primary care, with same or next-day appointments available seven days a week.
- Comprehensive Services: Onsite laboratory and community pharmacy for enhanced convenience and efficiency.
- Commitment to Health Equity & Community:
- Serve diverse populations through our FQHC locations, all recognized as Patient-Centered Medical Homes (PCMH), offering sliding-scale fees and care for everyone, regardless of their ability to pay.
- Engage in community health initiatives, wellness programs, and projects aimed at improving population health.
