Primary Care Physician (Internal Medicine or Family Medicine) — ISNP Program (Las Vegas, NV)

Full-Time | Institutional Special Needs Plan (ISNP) | Skilled Nursing & Long-Term Care

Our client is seeking a Board-Eligible or Board-Certified Internal Medicine or Family Medicine Physician to join its growing Institutional Special Needs Plan (ISNP) team in Las Vegas, Nevada. This is an outstanding opportunity to provide value-based, patient-centered primary care to medically complex seniors living in skilled nursing facilities, long-term care communities, and other institutional settings.

Physicians work alongside an experienced interdisciplinary team to improve patient outcomes, reduce unnecessary hospitalizations, and deliver proactive, compassionate care to one of the most vulnerable patient populations.

Compensation & Benefits

  • Base Salary: $211,369 – $317,053 annually
  • Health, Dental & Vision Insurance (effective the first day of the month following your start date)

Schedule

  • Full-Time
  • Primarily facility-based within Skilled Nursing and Long-Term Care settings
  • Travel between assigned facilities as needed
  • Collaborative team-based schedule focused on continuity of care

Responsibilities

  • Provide comprehensive primary care services to medically complex senior patients enrolled in the ISNP program
  • Conduct admission assessments, routine follow-up visits, and acute evaluations as clinically appropriate
  • Develop individualized treatment plans focused on chronic disease management and preventive care
  • Coordinate care with nurses, care managers, specialists, social workers, facility staff, and other healthcare professionals
  • Manage transitions of care following hospitalizations and post-acute admissions
  • Perform medication reconciliation and reduce avoidable readmissions
  • Educate patients and families regarding treatment plans, preventive care, and advanced care planning
  • Maintain accurate and timely EMR documentation
  • Complete documentation supporting quality initiatives, risk adjustment, and CMS compliance
  • Participate in interdisciplinary case conferences and quality improvement initiatives
  • Support evidence-based, value-driven care while improving patient outcomes and overall quality of life

Requirements

  • MD or DO degree
  • Board Certified or Board Eligible in Internal Medicine or Family Medicine
  • Completion of an accredited residency program
  • Active, unrestricted Nevada medical license (or ability to obtain licensure)
  • Current DEA registration
  • Strong commitment to patient-centered, value-based care

Preferred Qualifications

  • Experience caring for seniors or medically complex patient populations
  • Experience practicing in Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Assisted Living, or post-acute settings
  • Knowledge of Medicare Advantage, HEDIS, STAR measures, value-based care, and HCC documentation
  • Excellent communication and interdisciplinary collaboration skills
