Join a well-established primary care practice group with 50+ providers in 5 clinic locations serving Jacksonville, FL. National value-based managed care practice group. Physician-led focus on quality patient centered models. Top 10 rating for Medicare Advantage.  Florida has no state tax.  

Collaborative teams!  Established patient panel supporting a diverse community.  Support adults and geriatric patients with preventative and chronic care management.  Full service including in-house labs, diagnostic testing, and imaging. Solid mid-level and medical staff.  5 day schedule (Monday - Friday), no evenings, no weekends.  Average 18-21 patients p/day.  Call shared between providers (phone consults only).  Athena EMR.  

Requirements:

• BE/BC
• Family Medicine or Internal Medicine or Geriatric Medicine
• Managed Care (Medicare Advantage) experience
• Bilingual (Spanish) proficiency a plus!
• FL state medical license, or eligible

Compensation / Benefits:

Anticipated annual earning (low $300Ks)

• Guaranteed base salary (up to $270k)
• Production Incentives (up to 25% of base) paid quarterly
