Compensation
- Total earning potential: Up to $312,000
- Annual bonus up to 20% of base salary

Practice Details- Outpatient Primary Care
- Value-Based Care model
- Patient population: Seniors with 3 5 chronic conditions (heart disease, diabetes, hypertension, etc.)
- 15 18 patients per day
- Maximum panel size of 650 patients
- Shared call schedule based on market provider count (example: 1:32 weeks if 32 physicians)
- Dedicated support team for each physician, including:
- Medical Assistant
- Scheduler
- Referral Specialist
- Leadership support
Requirements- MD or DO
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Geriatrics fellowship preferred, but not required
- Current, unrestricted medical license
- Current DEA registration
