About the Role: As a Primary Care Physician within our PACE program, you will work closely with an interdisciplinary team to coordinate and deliver individualized care plans for seniors with complex medical and social needs. This model allows you to build deeper relationships with patients and directly see the impact of your care. Physicians primarily work onsite at the center, with one day typically dedicated to home visits and visits to senior living facilities.

Interview Process:
