As a Primary Care Physician, you will provide comprehensive outpatient care with an emphasis on preventive medicine, chronic disease management, wellness, and building long -term relationships with patients. Responsibilities<\/span><\/span><\/b>
<\/p>

• Provide comprehensive outpatient primary care services to adult and geriatric patients.<\/span>
  <\/li>
• Diagnose, treat, and manage acute and chronic medical conditions.<\/span>
  <\/li>
• Develop individualized care plans that promote preventive care, wellness, and disease management.<\/span>
  <\/li>
• Coordinate care with specialists, hospitals, and interdisciplinary care teams.<\/span>
  <\/li>
• Educate patients and caregivers on treatment plans, preventive health, and healthy lifestyle choices.<\/span>
  <\/li>
• Maintain accurate and timely medical documentation within the electronic medical record (EMR).<\/span>
  <\/li>
• Participate in quality improvement initiatives focused on value -based care and positive patient outcomes.<\/span>
  <\/li>
• Build strong, long -term relationships with patients and their families through compassionate, patient -centered care.<\/span>

  Qualifications<\/span><\/span><\/b>
  <\/p>

  • MD or DO from an accredited medical school.<\/span>
    <\/li>
  • Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.<\/span>
    <\/li>
  • Active Florida medical license (or eligible for licensure).<\/span>
    <\/li>
  • DEA license (or eligible).<\/span>
    <\/li>
  • Current BLS certification.<\/span>
    <\/li>
  • Experience with Medicare Advantage, managed care, value -based care, or geriatric populations is preferred but not required.<\/span>
    <\/li>
  • New graduates are encouraged to apply.<\/span>
    <\/li>
  • Bilingual (Spanish) skills are preferred for select locations.<\/span>
