Practice Highlights & Innovations
- Team-Based, Integrated Care:
- Our RN Care Managers assist all providers in managing risk-stratified patient populations, ensuring coordinated care for those most in need.
- Embedded Behavioral Health:
- Onsite support from Social Workers, Licensed Clinical Workers, Psychiatrists, and Advanced Practice Nurses offers comprehensive behavioral health services.
- Collaborative Hospitalist Support:
- Experience seamless transitions and support for patients requiring inpatient care.
- Patient-Centered & Accessible:
- Provide flexibility with both in-person and virtual care options, including same or next-day appointments available seven days a week.
- Access to on-site laboratory and community pharmacy enhances convenience and efficiency.
- Commitment to Health Equity & Community:
- Serve diverse populations through our FQHC locations, recognized as Patient-Centered Medical Homes (PCMH), offering sliding-scale fees and care for all, regardless of financial ability.
- Engage in community health initiatives, wellness programs, and projects aimed at improving population health.
