Job Overview
We are seeking a Primary Care Physician, Pod Leader to provide comprehensive, patient-centered care within a value-based healthcare model focused on senior populations. This role involves leading interdisciplinary care teams while delivering holistic primary care services to patients in community-based settings.
The physician will build long-term patient relationships, coordinate care plans, and collaborate closely with interdisciplinary team members to improve patient outcomes and quality of life.
Shift Details
Shift Time: 08:00 17:00
Schedule: Monday Friday
On-Call Requirement: Rotating phone-only on-call schedule a few nights/weekends per month
Number of Shifts: 5 shifts per week
Key Responsibilities
Perform comprehensive patient assessments and evaluations
Lead and collaborate with interdisciplinary care teams
Develop individualized care plans for senior patients
Coordinate acute, short-term, and long-term care transitions
Provide comprehensive primary care services in clinic and community settings
Support Nurse Practitioners and Physician Assistants within the care team
Maintain accurate patient documentation and compliance standards
Focus on preventative and holistic patient care approaches
Requirements
Completion of an accredited residency program
Doctorate Degree required
Active unrestricted state physician license
State CDS certification and DEA registration required
Board Certification preferred
CPR and First Aid certification required within 180 days of hire
2 5 years of primary care experience preferred
Experience working within an interdisciplinary team structure preferred
