Primary Care Physician (Geriatrics / Family / Internal Medicine)

Role Summary

Duties & Responsibilities

Comprehensive Clinical Care

  • Holistic Assessments: Perform thorough, in-person clinical assessments to evaluate the physical, mental, and psychosocial needs of frail elderly patients.

  • Care Plan Development: Partner with and lead a multidisciplinary team to build, review, and adjust deeply personalized care plans for each senior.

  • Manageable Patient Volumes: Focus entirely on high-quality, comprehensive care with an average caseload of just 8 patients per day—giving you the time required to treat the whole person.

  • Community Outreach: Provide a blend of clinic-based medical care and impactful home visits to meet patients where they are most comfortable.

  • Continuum of Care: Oversee and coordinate safe discharges for acute, short-term, and long-term placement needs.

  • Mid-Level Collaboration: Provide clinical leadership, guidance, and oversight to Nurse Practitioners (NPs) and Physician Assistants (PAs) within the clinic infrastructure.

  • Dedicated Outpatient Focus: Manage care completely within the community setting—no hospital rounds required.

  • Other duties as assigned.

Required Qualifications

  • Education: Doctorate degree (MD or DO) from an accredited medical school.

  • Residency Training: Successful completion of a medical residency program accredited by the Accreditation Council for Graduate Medical Education (ACGME).

  • Licensure: An active, unrestricted State Physician License with full eligibility to enroll as a provider for Medicare and Medi-Cal/Medicaid patients.

  • Registrations: Active state CDS certification and valid DEA registration without restrictions.

  • Life Support: Ability to obtain CPR and First Aid certifications within 180 days of your hire date (certifications in-hand upon hire preferred).

Desired Qualifications

  • Clinical Experience: 2 to 5 years of post-residency primary care experience, with a strong, dedicated focus on geriatric care or complex chronic disease management.

  • Board Certification: Board Certified or Board Eligible in Internal Medicine, Family Medicine, Geriatric Medicine, or Emergency Medicine is highly preferred.

  • Team Structure: Prior professional experience collaborating successfully within a team framework.

Location and Work Type

  • Work Type: Full-Time, Permanent Position.

  • Work Setting: On-site clinical environment integrated with community home visits.

  • Schedule: Balanced lifestyle schedule consisting of Monday through Friday, 8:00 AM – 5:00 PM (weekends off).
