Healthcare United is seeking a Lead Primary Care Physician<\/b> for outpatient, value -based primary care opportunities in Columbus and Cincinnati, Ohio<\/b>.
<\/p>
This is an excellent opportunity for a Family Medicine or Internal Medicine physician<\/b> who is interested in a mission -driven primary care model with smaller patient panels, longer patient visits, strong team support, and a focus on improving outcomes for medically complex patient populations.
<\/p>
The physician will provide direct outpatient primary care while also serving as a clinical leader within an interdisciplinary care team. This role is ideal for a provider who enjoys patient care, team collaboration, population health, chronic disease management, and helping reduce avoidable ER visits, hospitalizations, and readmissions.
<\/p>
The care model is built around whole -person care, strong provider support, care coordination, behavioral health, pharmacy, social work, nursing, and clinical leadership.

Job Responsibilities<\/b>
<\/p>- Provide outpatient primary care to adult patients in a value -based care setting
<\/li> - Manage chronic conditions, preventive care, acute visits, post -hospital follow -up, and ER follow -up
<\/li> - Lead and collaborate with an interdisciplinary care team
<\/li> - Support APPs, nurses, behavioral health, pharmacy, social work, nutrition, and care management teams
<\/li> - Help develop care plans for medically complex patients
<\/li> - Participate in population health huddles and quality improvement initiatives
<\/li> - Work to reduce avoidable ER visits, hospitalizations, and readmissions
<\/li> - Support transitional care planning across the continuum of care
<\/li> - Provide clinical leadership and mentorship to providers and staff
<\/li> - Use data, EHR tools, and quality metrics to improve patient outcomes
<\/li> - Serve as a clinical leader within the local market

Requirements<\/h3>- MD or DO degree required
<\/li> - Board Certified or Board Eligible in Family Medicine<\/b> or Internal Medicine<\/b>
<\/li> - Active or eligible for an unrestricted Ohio medical license
<\/li> - Primary care, outpatient medicine, community health, FQHC, or value -based care experience preferred
<\/li> - Clinical leadership experience preferred
<\/li> - Experience mentoring physicians, APPs, or care teams preferred
<\/li> - Comfortable caring for medically complex and underserved patient populations
<\/li> - Strong communication, leadership, organization, and problem -solving skills
<\/li> - Ability to work in a team -based, outcomes -driven care model
