The Primary Care Physician will provide comprehensive care to patients across the lifespan, including preventive services, chronic disease management, and acute care. Key Responsibilities:

  • Provide comprehensive primary care services including physical exams, diagnosis, and treatment planning
  • Manage acute and chronic conditions such as diabetes, hypertension, and respiratory illnesses
  • Perform routine procedures and preventive screenings
  • Prescribe medications and therapies in accordance with state regulations
  • Collaborate with specialists and care teams for coordinated patient care
  • Educate patients on health maintenance, disease prevention, and lifestyle modifications
  • Maintain accurate and timely documentation within the EMR
  • Participate in quality improvement initiatives and population health programs
