What You Will Be Doing<\/span><\/b><\/span>
<\/div>- Perform comprehensive in -person assessments for senior participants.
<\/span><\/li> - Lead and partner with the interdisciplinary care team to develop individualized care plans.
<\/span><\/li> - Provide thoughtful primary care with an average of approximately eight patients per day.
<\/span><\/li> - Coordinate care transitions, including acute, short -term, and long -term placements.
<\/span><\/li> - Partner with Nurse Practitioners, Physician Assistants, nursing, social work, therapy, and other care team members.
<\/span><\/li> - Participate in regular care planning discussions focused on whole -person health.
<\/span><\/li> - Support home -based care needs through scheduled home visits as part of the role.
<\/span><\/li> - Participate in a rotating phone -only call schedule a few nights or weekends per month.
<\/span><\/li><\/ul>Schedule and Work Environment<\/span>

Regular business hours, typically 8:00 AM to 5:00 PM.

Weekends off, outside of limited rotating phone -only call responsibilities.

Requirements<\/h3>- Physician trained in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics.
<\/span><\/li> - Successful completion of an ACGME -accredited residency program.
<\/span><\/li> - Active state physician license without restrictions.
<\/span><\/li> - State CDS certification and DEA registration.
<\/span><\/li> - Eligible to provide services to Medicare and Medi -Cal participants.
<\/span><\/li> - Board certification preferred.
<\/span><\/li> - CPR and First Aid certification preferred upon hire, or ability to obtain within required timeframe.
<\/span><\/li> - Experience in geriatric care, senior care, value -based care, or complex primary care strongly preferred.
<\/span><\/li> - Experience working in an interdisciplinary team model preferred.
<\/span><\/li> - Experience leading or collaborating closely with Nurse Practitioners or Physician Assistants preferred.
