Trinity Health PACE is seeking a part-time (20 hours/week) Family Medicine, Internal Medicine, or Geriatric Medicine physician to provide services at Mercy LIFE Assisi House, Aston PA, and Mercy Life Valley View, Elwyn located in greater Philadelphia.

Under the supervision of the medical director, the Primary Care physician (PCP) provides primary medical care to PACE Organization (PO) participants, including assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct participant medical care, and evaluation of effectiveness of the plan of care. Functions in a collegial relationship with the other health care professionals, making independent decisions regarding medical needs and health regimens. Carries out medical procedures as indicated.

Position Details:

Mercy LIFE Assisi House is the retirement convent for the Sisters of St. Francis of Philadelphia. It is located across the street from Our Lady of Angels Convent, the congregations motherhouse.  A person-centered care approach fosters a sense of family throughout the 125 bed, multi-level care facility, which includes an Alzheimers and Dementia unit.

Mercy Life Valley View provides both housing and the Mercy LIFE PACE program for senior adults. This program provides services to a community specifically comprised of deaf seniors.  Candidates must be willing to learn American Sign Language.

This position will serve two 8 hour days at Assisi House and one 4 hour day at Valley View.  Days worked during the week are flexible.

No night, weekend, on call or holidays coverage is required with this position!

What you will do:

  • Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments.  Report changes in participants baseline status to appropriate staff on a daily basis.
  • Provide initial and ongoing participant assessments, care plan development, and implementation of care plan. Integrate the primary care treatment plan into the overall plan of care developed by the interdisciplinary team.
  • Refer participants to medical specialist as indicated. Manages all care of participants in the nursing home: Provides regular visits to manage the care of participants in the nursing home in collaboration with the Interdisciplinary Team; maintains communication with nursing home staff for continuous monitoring of PACE participants on a case-by-case basis; admits nursing home participants to hospital when necessary.
  • Oversees and manages participants use of medical specialist and inpatient care. Communicates and follows up with specialist regarding outcome of visit.
  • Review medical outcomes to establish best practices and improve participant health. Complete documentation and uses appropriate diagnostic coding.
  • Advise the Medical Director in ways and means to establish better accountability of PO services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands.
  • Assist with the development of policies and procedure, standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
  • Participate in provider on-call rotation as required.

Minimum Qualifications:

  • Doctorate Degree (Includes PhDs): MD or DO. Current and active plenary physician license to practice in state of PO required. CDS Certification and DEA Registration required.
  • Current BLS required.
  • Board Certification Internal Medicine or Family Practice required. Board Certification in Geriatrics preferred.
  • Must be willing to learn American Sign Language.
  • A minimum of one (1) year working with the frail or elderly population required
  • Must have a valid drivers license, proof of insurance and use of personal vehicle required.
  • Strong communication, problem-solving, and organizational skills. Able to communicate effectively with individuals and groups representing diverse perspectives.
  • Proficiency with Microsoft Office suite
  • Ability to change and adapt to changing priorities.
  • Must exhibit critical thinking skills and possess the ability to prioritize workload.
  • Periodic travel within the Center's geographic region.
