We’re currently recruiting for a Primary Care Physician in New York, NY with a mission-driven outpatient clinic focused on providing comprehensive, whole-person care to patients who often face significant barriers to consistent healthcare.

Job Details
Key Responsibilities:
Provide comprehensive primary care, including patient consultations, evaluations, diagnosis, treatment, and preventive care.
Develop individualized patient management plans based on medical history, physical assessments, and patient needs.
Diagnose and manage chronic medical conditions.
Focus on disease prevention, health promotion, and long-term patient wellness.
Provide and monitor medication-assisted treatment when appropriate.
Monitor medication therapies and adjust treatment plans based on patient response.
Integrate behavioral health considerations into primary care.
Collaborate closely with mental health professionals and other members of the care team.
Maintain accurate and thorough patient documentation within the EMR.
Work collaboratively with an interdisciplinary healthcare team to coordinate patient care.
Educate patients and families about medical conditions, treatment plans, medications, and healthy lifestyle choices.
Stay current with developments in primary care, family medicine, healthcare technology, and evidence-based practices.
Contribute to the development and improvement of clinical workflows and care processes as the clinic grows.
Experience & Qualification:
MD or DO degree from an accredited medical school.
Board Certified or Board Eligible in Family Medicine.
Valid and unrestricted New York medical license.
2+ years of primary care experience.
Experience providing primary care to adult populations.
Strong background in:
Chronic disease management
Preventive care
Health promotion
Medication management
Medication-assisted treatment
Behavioral health integration
Experience with Electronic Medical Records (EMR).
