Senior Doc is hiring a Primary Care Physician (MD/DO) to deliver mobile, home-based primary care to older adults across home, assisted living, and SNF settings. Ideal for physicians who want meaningful patient relationships, flexibility, and a value-based care model.

What You'll Do:

• Provide mobile primary care to seniors
• Manage a patient panel
• Treat chronic and complex geriatric conditions
• Collaborate with psychiatry, neurology, palliative care & care coordinators
• Maintain EMR documentation and support quality initiatives

Requirements:

• MD/DO; BC/BE in IM, FM, or Geriatrics
• Active state license (or ability to obtain)
• DEA license

Preferred: Experience in home-based primary care, SNF/ALF, geriatrics, post-acute care, or value-based care.
