We are seeking a compassionate and dedicated Primary Care Physician to deliver high -quality, value -based care to an established panel of predominantly geriatric patients. This role emphasizes preventive medicine, chronic disease management, and building meaningful, long -term patient relationships in a supportive outpatient environment.<\/span>

You will work within a collaborative, multidisciplinary care model designed to improve patient outcomes, enhance access to care, and reduce unnecessary hospitalizations. Our approach prioritizes personalized care plans, proactive health management, and patient education.<\/span>

Key Responsibilities<\/span><\/span><\/b>
<\/p>- Provide comprehensive primary care services, including evaluation, diagnosis, and treatment of acute and chronic conditions.<\/span>
<\/li> - Deliver preventive care, wellness visits, and health screenings with a focus on early intervention.<\/span>
<\/li> - Develop and manage individualized care plans for a predominantly geriatric population.<\/span>
<\/li> - Collaborate closely with nurse practitioners, care coordinators, and support staff to ensure seamless, patient -centered care.<\/span>
<\/li> - Utilize electronic health records (EHR) to document clinical encounters and track patient outcomes.<\/span>
<\/li> - Engage in quality improvement initiatives and contribute to a culture focused on continuous care enhancement.<\/span>

Qualifications<\/span><\/span><\/b>
<\/p>- MD or DO from an accredited medical school<\/span>
<\/li> - Internal Medicine or Family Medicine or Geriatric Medicine<\/span>
<\/li> - Active, FL license<\/span>
<\/li> - Current DEA registration<\/span>
<\/li> - At least 3 years of experience in Outpatient Primary Care<\/span>
<\/li> - Strong clinical judgment with a focus on preventive and longitudinal care<\/span>
<\/li> - Excellent interpersonal and communication skills<\/span>
<\/li> - Commitment to a team -based, patient -first care model<\/span>
