Summary of Services

The Primary Care Physician covers the range of primary care services as would be provided in a state-of-the-art health care treatment facility, and the standard of care shall be of a quality meeting or exceeding currently recognized national standards as established by:

• The American Board of Family Medicine.
• The American Board of Internal Medicine.
• The standards of the American Hospital Association.

Working Hours

Primary Care Physician will typically work Monday through Friday, 8:00 AM to 4:30 PM. The contractor should not exceed 40 hours per week.

Qualifications

• Hold a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia,
• Be Board Certified/Board Eligible in Internal Medicine or Family Medicine.
• Certification in Advanced Cardiac Life Support (ACLS).
• Certification in Cardio-Pulmonary Resuscitation (CPR).

Duties and Responsibilities

• Provide comprehensive primary care services to an assigned panel of Veterans, including preventive, acute, and chronic disease management.
• Perform complete history and physical examinations and develop appropriate differential diagnoses and treatment plans.
• Diagnose, treat, and manage acute illnesses, chronic medical conditions, and complex multisystem diseases commonly encountered in adult primary care.
• Order, interpret, and evaluate laboratory studies, diagnostic imaging, and other diagnostic procedures necessary for patient assessment and treatment.
• Prescribe medications, durable medical equipment, therapeutic interventions, and other treatments in accordance with VA clinical guidelines and applicable federal regulations.
• Manage chronic diseases including, but not limited to:

  • Diabetes mellitus
  • Hyperlipidemia
  • Coronary artery disease
  • Congestive heart failure
  • Chronic obstructive pulmonary disease (COPD)
  • Asthma
  • Chronic kidney disease
  • Obesity
  • Thyroid disorders
  • Arthritis and chronic pain
• Provide preventive healthcare services including routine health maintenance, cancer screenings, immunizations, tobacco cessation counseling, weight management, and lifestyle modification counseling.
• Conduct annual wellness visits and comprehensive preventive health evaluations consistent with VHA preventive care initiatives.
• Identify and manage behavioral health concerns including depression, anxiety, PTSD, substance use disorders, insomnia, and suicide risk while coordinating care with VA Mental Health services.
• Collaborate with Behavioral Health providers utilizing the VA Primary Care–Mental Health Integration (PCMHI) model.
• Coordinate specialty referrals and consultations with VA specialists and community care providers when clinically indicated.
• Participate in interdisciplinary care planning with nurses, pharmacists, social workers, behavioral health providers, dietitians, and other healthcare professionals.
• Utilize the VA Patient Aligned Care Team (PACT) model to ensure coordinated, team-based delivery of healthcare services.
• Develop individualized treatment plans that incorporate the Veteran's goals, preferences, functional status, and overall health needs.
• Perform office-based procedures within credentialing and privileging guidelines, including but not limited to:
• Skin biopsies
• Joint injections
• Cryotherapy
• Incision and drainage of minor abscesses
• Removal of benign skin lesions
• Wound evaluation and management
• Provide appropriate follow-up care after hospitalizations, emergency department visits, specialty consultations, and diagnostic testing.
• Review abnormal diagnostic findings promptly and initiate appropriate interventions.
• Monitor medication effectiveness, adverse drug reactions, polypharmacy risks, and medication compliance.
• Educate Veterans and family members regarding diagnoses, treatment options, medication management, disease prevention, and self-management strategies.
• Promote patient engagement through shared decision-making and Veteran-centered communication.
• Maintain complete, accurate, and timely electronic medical documentation using the VA Computerized Patient Record System (CPRS) and transition workflows supporting the Federal Electronic Health Record (EHR) system, in accordance with VHA documentation standards.
• Complete all clinical documentation, encounter coding, consults, orders, and administrative actions within established VA performance timelines.
• Participate in quality improvement initiatives, peer review activities, performance measurement programs, and patient safety initiatives.
• Monitor clinical quality indicators and participate in efforts to improve patient outcomes, access to care, and operational efficiency.
• Support VA population health initiatives including disease registries, preventive screening compliance, chronic disease monitoring, and care gap closure.
• Respond appropriately to urgent and same-day patient care needs, triaging Veterans based on clinical urgency and coordinating emergency services when required.
• Comply with all VA policies regarding infection prevention, patient safety, privacy, HIPAA, information security, controlled substance prescribing, and ethical medical practice.
• Maintain unrestricted medical licensure, DEA registration (as applicable), Basic Life Support (BLS) certification, credentialing, privileging, and continuing medical education requirements.
• Participate in staff meetings, clinical case conferences, continuing medical education, and mandatory VA training.
• Collaborate with leadership to support clinic performance metrics related to access, quality, patient satisfaction, continuity of care, and Veteran experience.
• Maintain professional communication and effective working relationships with Veterans, family members, caregivers, interdisciplinary staff, community providers, and VA leadership.
• Perform other clinical and administrative duties within the scope of practice as assigned by the Chief of Primary Care or designated VA leadership in support of the mission of the Veterans Health Administration.
