Why This Role is Different

In this role, you aren't just treating symptoms—you are managing lives. You will serve as the clinical leader of an interdisciplinary team, collaborating with nurses, social workers, and therapists to create comprehensive care plans that keep patients healthy, independent, and in their homes.

  • Manageable Caseload: See only 8–10 patients per day, allowing for deep clinical assessment and relationship building.
  • True Work-Life Balance: Maintain a standard 8:00 AM – 5:00 PM, Monday through Friday schedule. Enjoy your weekends and evenings with a phone-only, limited on-call rotation.

Core Responsibilities

  • Clinical Excellence: Conduct thorough in-person assessments and provide high-touch primary care.
  • Collaborative Leadership: Lead Interdisciplinary Team (IDT) meetings to craft and execute personalized, holistic care plans.
  • Care Coordination: Oversee transitions of care, including acute episodes and long-term placement management.
  • Advocacy: Act as the primary advocate for your patients within the Medicare and Medi-Cal systems.

Who You Are

We are looking for compassionate clinicians who are passionate about the future of value-based medicine. This is an ideal environment for both seasoned experts and new residency graduates looking to make a lasting impact.

  • Education: Completion of an accredited residency in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics.
  • Licensure: Active, unrestricted state physician license, CDS certification, and DEA registration.
  • Certification: Board Certification is preferred; ability to maintain CPR and First Aid certification is required.
  • Experience: 2–5 years of primary care experience is ideal, but passionate new graduates are encouraged to apply.
  • Eligibility: Must be eligible to serve Medicare and Medi-Cal participants.
