Performance Objectives
Build and retain a full panel by delivering continuity that earns trust and return visits.
Close care gaps so eligible patients complete evidence-based screenings, immunizations, and chronic-disease monitoring on schedule.
Move hypertension, diabetes, and other common conditions toward guideline targets, with documented improvement among uncontrolled patients.
Resolve most acute and same-day needs in the medical home, reducing avoidable emergency and specialist use.
Lead nursing, medical assistants, and care coordination so visits, refills, and results do not stall.
Complete clear same-day documentation that supports safe handoffs, accurate coding, and quality reporting.
Set plans patients can follow, including behavioral health and social needs that change clinical results.
Environment & Resources
You will practice in outpatient primary care with a dedicated care team, an electronic health record, and support for labs, referrals, and coordination. Quality dashboards, clinical protocols, and protected time for panel work are part of the role. You report to the medical director and work with specialists as partners, not as a solo practitioner.
Essential Qualifications
MD or DO, board-certified or board-eligible in family medicine or internal medicine.
Active, unrestricted medical license and DEA registration, or the ability to obtain both before start.
Proven management of a primary care panel, including prevention and chronic disease.
Ability to lead a team-based clinic and finish same-day documentation in an EHR.
Clear communication with patients across ages, cultures, and levels of health literacy.
