The role involves delivering care to Veteran patients, demonstrating exceptional clinical judgment, medical expertise, and professional skills, including medical interviews, physical examinations, and procedural techniques.
This position is situated in an outpatient clinic where the primary care physician typically sees between 12 to 14 patients daily.
Utilizing a personalized, patient-focused approach, the physician will consider the Veterans' objectives, diagnoses, and their physical, functional, and psychosocial requirements.
In emergency situations, the physician will initiate and manage treatment and transfers as necessary.
The physician will oversee a designated panel of patients while also providing support for other Veterans enrolled in primary care at the clinic.
Referrals to appropriate specialists and healthcare facilities will be made as needed, along with coordination with home health services.
Documentation of assessments and care plans for each patient must align with established standards, policies, and procedures.
Proficiency in Tele-Health services, including home visits via platforms like VA Video Connect, is expected.
Assistance in coordinating care received from non-VA healthcare providers in the community will also be part of the responsibilities.
Maintaining state licensure, PECOS enrollment, and staying updated on current clinical practices is essential.
The clinic operates under the VA Patient-Aligned Care Team model, which includes a dedicated Registered Nurse Care Coordinator, Clinical Associate (such as an LPN), and Administrative Associate (like a Clerk). Additional support is provided by a Clinical Pharmacy Practitioner, Social Worker, and Mental Health staff for the care of enrolled Veterans.
