Physician – Post-Acute Care (Riverdale - Bronx, NY)

We are hiring a full-time, quality-focused physician to lead clinical care across skilled nursing facilities in the Bronx, NY. This role offers high clinical autonomy, robust operational backing, and uncapped, fee-for-service earning potential.

What You Will Do

• Patient Care: Deliver comprehensive primary and post-acute care to a diverse patient community (Monday–Friday rounding).

• Collaboration: Partner with interdisciplinary facility teams, patients, and families to manage care transitions and treatment plans.

• Quality Improvement: Utilize data-driven population health tools and participate in value-based shared savings programs to optimize outcomes.

Requirements

• MD or DO with valid New York medical license (or eligibility).

• Board certification or eligibility in Internal Medicine, Family Medicine, or Emergency Medicine

• Strong diagnostic, communication, and interdisciplinary collaboration skills.
