Primary/Family Nurse Practitioner (NP) – PRN
Hours: Approximately 24–32 hours per week; hours may vary
Schedule: Monday–Friday, between 8:00 AM–5:00 PM
Weekends/Holidays: No weekends or holidays
Position Summary
We are seeking a Primary/Family Nurse Practitioner (NP) to provide comprehensive primary and family healthcare services. This position is not a mental health role.
The Nurse Practitioner will evaluate and manage acute and chronic medical conditions, develop and implement patient care plans, prescribe medications and treatments within the scope of licensure, order and interpret diagnostic testing, and provide clinical direction to nursing and other healthcare staff.
Key Responsibilities1. Patient Care & Clinical Management
Create and implement patient care plans and perform advanced nursing duties within the scope of licensure.
Monitor patients' progress and intervene in the management of chronic illnesses.
Perform routine physical examinations to assess chronic conditions and diagnose new conditions.
Evaluate acute and chronically ill patients and prescribe medications or administer appropriate treatments.
Complete discharge planning and follow-up care for patients with chronic care and/or medical issues.
Perform minor surgical procedures as permitted by training and demonstrated competency.
Evaluate the need for referral and consultation inside or outside the facility.
Order and review laboratory, X-ray, and other diagnostic testing.
Provide immunizations and vaccinations against communicable diseases.
2. Medical Records & Documentation
Maintain accurate and complete medical records.
Document medical information obtained during patient interviews in accordance with agency guidelines, policies, protocols, and professional standards.
Document assessments, diagnostic impressions, treatment plans, and follow-up parameters.
Cooperate with the Chief Medical Officer (CMO) or agency equivalent on medical quality oversight.
3. Clinical Leadership
Provide clinical direction to Registered Nurses, Licensed Practical Nurses, phlebotomists, and other caregivers.
Serve as a clinical resource for patients and families regarding health maintenance, health promotion, and identification of health risk factors.
4. Meetings, Quality Improvement & Professional Development
Attend facility-specific professional meetings as requested.
Participate in continuous quality improvement initiatives and monthly meetings.
Attend quarterly medical meetings.
Participate in professional and agency committees as required.
Attend in-services and continuing education opportunities related to professional growth and development as a Nurse Practitioner.
Required Knowledge & Skills
Knowledge of skilled registered nursing procedures and advanced nursing practice.
Knowledge of HIPAA regulations and applicable state and federal nursing regulations, policies, and procedures.
Strong clinical knowledge and judgment to make appropriate decisions regarding patient care and treatment.
Knowledge of healthcare services and program management.
Strong communication, interviewing, and interpersonal skills.
Ability to handle sensitive telephone and face-to-face interactions professionally.
Ability to gather, organize, and evaluate information; identify problems; establish facts; and reach valid conclusions.
Ability to understand and follow facility philosophies, policies, and procedures.
Ability to establish and maintain positive relationships with patients, families, nursing staff, and other facility/agency personnel.
Ability to maintain strict confidentiality of patient information.
Proficiency with standard clinical equipment, including stethoscope, blood pressure cuff, EKG machine, otoscope, ophthalmoscope, reflex hammer, and surgical equipment as applicable.
Basic computer proficiency and ability to maintain electronic medical records.
Minimum Qualifications
Valid Registered Nurse (RN) license.
Completion of an accredited Nurse Practitioner program.
Certified by the Ohio Board of Nursing for advanced practice.
Valid Certificate of Authority.
Valid Certificate to Prescribe in accordance with Ohio Revised Code (ORC) 4723.
Ideal Candidate
The ideal candidate is an experienced Primary Care or Family Nurse Practitioner with a strong clinical foundation and the ability to independently evaluate, diagnose, treat, and manage patients with both acute and chronic medical conditions.
This position is specifically for Primary/Family Practice and is not intended for Mental Health/Psychiatric Nurse Practitioners.
