Private Duty Nurse - Advanced Dementia Client

Private Duty Nurse needed with experience in Advanced Dementia. Home Care client needs a Licensed Practical Nurse for his ongoing schedule. (Could Accept a Registered Nurse, Inquire at Interview) 

Shift Need: (You can cover all or some!) If covering all = 34 Hours per week 

• Fridays and Sundays 7am-7pm
• Tuesdays 1-7pm
• Thursdays 7pm-11pm

(Additional client shifts available if requested) 

Pay: $35-$43/Hour

Nurse Job Duties:

• Perform care as defined by client’s individual Plan of Care and within scope of state’s nurse practice act.
• Monitor patient’s condition and notify appropriate personnel of any changes.
• Provide those services requiring substantial and specialized nursing skill.
• Assures proper maintenance of clinical records in compliance with local, state and federal laws.
• Counsels the client and family in meeting nursing and related needs.

Position Requirements 

Nurse Qualifications:

• Current and unrestricted RN or LPN license in state (Illinois) for which you are applying.
• Home Health Experience
• Minimum 2 years experience as a nurse.
• Proof of eligibility to work in the US.
• Reliable car / auto insurance.
• Clean background and criminal record.
