Essential Duties and Responsibilities
The essential functions include, but are not limited to, the following:
- Deliver direct patient care as assigned, using established procedures in the patient’s home environment.
- Traveling to patients’ homes and managing their care plans according to physicians’ instructions
- Maintain accurate, detailed reports and records on medical information, vital signs, or changes in patient’s conditions.
- Monitor patients' medication usage and results.
- Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes
- Evaluate patients' behavior to formulate diagnoses or assess treatments.
- Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
- Provide personal assistance, medical attention, emotional support, or other personal care to patients.
- Coordinate with other home health professionals caring for the patient, and communicate the patient’s needs to the family providing support.
- Train other Nurses, coordinate with other home health professionals caring for the patient, and communicate the patient’s needs to the family providing support.
- Assures the continuity of care through the delivery of quality patient/client care.
- Provides effective communication to patient/client/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner
- Monitors assigned cases to ensure compliance with physician orders and requirements of third party payor.
- Demonstrates commitment, professional growth, and competency.
- Other duties as assigned.
