• Provide one on one care to a single client in their home setting and while in the community.
• Implements the client’s physician ordered plan of care and nursing interventions while adhering to agency policies.
• Perform assessments to monitor the health status of the client.
• Notify the physician and Field Nurse Case Manager of any changes in the client’s condition.
• Coordinate care with the Field Nurse Case Manager and other members of the client’s multidisciplinary care team.
• Administer medications and treatments as ordered.
• Document performed interventions and treatments.
• Assists clients with activities of daily living such as bathing, toileting, dressing, and grooming.
• Collaborate with the care team on implementation and updates to the client’s plan of care.
• Monitor client progress toward home health care goals.
• Educate the client and family regarding the plan of care and nursing care needs.

Strong communication skills. Strong computer skills.Licenses/Certifications: Colorado RN license in good standing with the Department of Regulatory Affairs. BLS required. Up to date car insurance and driver license will be required.
