Responsibilities:
Administer medications and monitor patient health as per care plans.
Provide essential nursing care and support to clients in their homes.
Assist with daily living activities, including bathing, dressing, and meal preparation.
Communicate effectively with clients, families, and healthcare teams.
Document patient care and progress accurately and in a timely manner.
Educate clients and families on health management and wellness.
Participate in care planning and contribute to team meetings.
Maintain a safe and clean environment for clients at all times.
Requirements:
Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Missouri.
Previous experience in home health care or private duty nursing preferred.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Compassionate demeanor with a focus on patient-centered care.
Basic Life Support (BLS) certification required.
Flexibility to work various shifts including weekends and PRN.
Reliable transportation and valid driver's license.
