Responsibilities

• Provide one-on-one skilled nursing care in the home setting
• Perform comprehensive nursing assessments
• Administer medications and treatments as ordered
• Perform skilled nursing procedures in accordance with physician orders and the plan of care
• Monitor skin integrity and implement pressure injury prevention measures
• Assist with positioning, mobility, and participant safety
• Observe, assess, and promptly report changes in condition
• Coordinate care with physicians, therapists, family members, and the interdisciplinary team
• Complete accurate, timely clinical documentation
• Maintain compliance with agency policies, state regulations, and nursing standards

Qualifications

• Current Pennsylvania Registered Nurse (RN) License
• Current CPR Certification
• Reliable transportation
• Strong assessment and critical thinking skills
• Excellent communication and documentation skills
• Home care, rehabilitation, long-term care, or private duty nursing experience preferred, but not required
