Overview

Heritage Home Healthcare is seeking a compassionate and dependable PRN Caregiver in the West Albuquerque, NM area (87121) to provide high-quality in-home care to clients. In this role, you will provide delegated personal care services in a home setting, supporting clients with activities of daily living while ensuring comfort, safety, and dignity. Key Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and eating
  • Follow individualized care plans and delegated care instructions provided by licensed professionals
  • Provide mobility assistance, including transfers, repositioning, and wheelchair support
  • Support medication reminders and delegated care tasks per care plan and agency guidelines
  • Maintain a clean, safe, and organized home environment through light housekeeping assistance
  • Provide companionship, emotional support, and encouragement to clients
  • Escort or transport clients to appointments or errands as needed
  • Observe, document, and report changes in client condition, behavior, or care needs to the clinical team
  • Maintain professionalism, confidentiality, and compassionate client-centered care at all times

Qualifications

  • Must be at least 18 years of age
  • High school diploma or GED preferred, but not required
  • Previous caregiving experience preferred
  • Ability to follow instructions and care plans accurately
  • Strong interpersonal and communication skills
  • Must have a current valid driver’s license
  • Reliable transportation and valid auto insurance required
  • Must live in Albuquerque, NM or surrounding areas

Schedule

  • PRN / as-needed scheduling
  • Flexible availability required
