    Requirements

    Key Responsibilities:

    • Conduct initial and ongoing assessments of patient needs.
    • Implement and adjust individualized care plans.
    • Ensure compliance with hospice policies and Medicare regulations.
    • Educate patients and families on self-care and end-of-life options.
    • Supervise paraprofessionals delivering patient care.
    • Document and report clinical findings and participate in interdisciplinary meetings.
    • Enhance patient satisfaction and experience.

    Qualifications:

    • Current Michigan LPN License in good standing.
    • Minimum of 1 year experience in hospice or palliative care.
    • Strong empathy and communication skills.
    • Flexibility to respond to varying patient needs.
