Flexible Schedule | Independent Contractor (1099) | Northern Virginia
About the Role:
Paragon Home Care is a non-medical home care agency serving older adults throughout Northern Virginia. We are seeking an experienced Registered Nurse (RN) to conduct in-home client assessments and develop individualized plans of care for our caregiving team. This is not a hands-on or bedside nursing position. Visits primarily involve assessing the client's needs, home environment, and safety. Most assessments take approximately 60 minutes, plus documentation.
Responsibilities:
- Conduct comprehensive per-visit nursing assessments including ADLs/IADLs, cognition, mobility, fall risk, home safety, medication self-administration, and dietary needs
- Develop, review, and update individualized plans of care that our caregivers will follow
- Develop, review, and update individualized care plans based on assessment findings
- Complete visits at required intervals and reassess clients when their needs or condition change
- Document assessments and care plans in AxisCare (training provided)
- Communicate relevant client needs or changes with our office team
Schedule & Pay:
- Set your own schedule and choose which assessments you accept.
- Visits are typically completed on weekdays during business hours.
- Flat fee per completed assessment.
- Independent Contractor (1099) position.
Requirements:
- Active Registered Nurse (RN) license in the state of Virginia
- Prior experience assessing older adults: home health, home care, or community nursing preferred
- Strong clinical assessment and critical thinking skills
- Excellent documentation skills
- Reliable transportation, valid driver's license, current auto insurance
- Ability to work independently with minimal supervision
- Strong interpersonal and communication skills with patients, families, and care teams
- Able to clear a Virginia criminal background check (required by state law for home care organizations)
- Must provide TB screening documentation
