PRN Home Health Speech-Language Pathologist (SLP)

Help Patients Find Their Voice, Confidence, and Independence at Home

Make a meaningful difference while enjoying the flexibility of a PRN schedule. As a Home Health Speech-Language Pathologist, you'll provide personalized, one-on-one care to patients in the setting where they feel most comfortable-their own homes. You'll assess and treat communication, cognitive, and swallowing disorders while educating patients and caregivers on techniques that support safety, independence, and quality of life.

What You'll Do

• Evaluate patients' speech, language, cognitive, communication, and swallowing needs.

• Develop individualized treatment plans based on physician orders and assessment findings.

• Provide skilled therapy services in patients' homes.

• Teach patients and caregivers exercises, techniques, and strategies to support continued progress.

• Recommend and provide instruction on assistive devices when appropriate.

• Monitor patient progress and update treatment plans as needs change.

• Communicate changes in condition, progress, and recommendations to physicians and the interdisciplinary care team.

• Coordinate services with nurses, therapists, caregivers, and other healthcare professionals.

• Complete accurate and timely clinical documentation.

• Maintain compliance with professional standards, regulatory requirements, and company policies.

What You'll Need

• Master's degree in Speech-Language Pathology from an accredited program.

• Current, unrestricted Speech-Language Pathologist license in the state of practice.

• At least one year of SLP experience in a clinical care setting.

• Valid driver's license and an acceptable driving record, or reliable transportation to travel between patient homes and assigned locations.

• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
