You participate in the development of the plan of care for each patient you work with while maintaining clinical and progress notes. Working alongside the family, you compile a list of community resources that are the most helpful for your patient while adhering to the plan of care. You also help with discharge planning and attend in-services as needed.In addition, you act as a consultant to other employees within our organization to ensure our patients are all receiving well-rounded care. The safety of your patients is of the utmost importance to you, and you take great care to practice infection control by following universal precautions. You also actively recognize and report any forms of abuse according to policy as well as federal and state laws.
