Responsibilities:
Provide skilled nursing care to senior clients in a home health setting
Administer medications, injections, and treatments as prescribed by physicians
Monitor and document patient health status, vital signs, and changes in condition
Develop and follow individualized patient care plans in coordination with the care team
Communicate effectively with patients, families, and healthcare providers regarding care needs
Perform wound care, catheter care, and other clinical procedures as needed
Ensure compliance with state regulations, agency policies, and homecare standards
Requirements:
Active Licensed Practical Nurse (LPN) license in the state of Virginia
Previous home health, geriatric, or senior care experience preferred
Strong clinical assessment and critical thinking skills
Ability to work independently with minimal supervision in a home environment
Excellent communication and interpersonal skills with patients and families
Valid driver's license and reliable transportation required
CPR/BLS certification required or ability to obtain upon hire
