About the Role: Right at Home Des Moines is looking for a compassionate and skilled PRN Occupational Therapist to join our dedicated home care team in West Des Moines, IA. In this flexible per-diem role, you'll make a meaningful difference by helping clients regain independence and improve their quality of life — right in the comfort of their own homes. Responsibilities:
Conduct comprehensive occupational therapy evaluations and assessments for home care clients
Develop and implement individualized treatment plans to improve clients' daily living and functional skills
Provide therapeutic interventions focused on ADLs, fine motor skills, cognitive rehabilitation, and home safety
Recommend and train clients and caregivers on adaptive equipment and assistive devices
Collaborate with interdisciplinary care teams including nurses, physical therapists, and care coordinators
Document patient progress, treatment plans, and clinical notes in compliance with regulatory standards
Educate clients and family members on home exercise programs and injury prevention strategies
Requirements:
Graduate of an accredited Occupational Therapy program (Master's or Doctoral degree preferred)
Active Iowa Occupational Therapist (OT) license in good standing
NBCOT certification required
Previous home health or home care experience preferred
Strong knowledge of ADL training, adaptive equipment, and functional rehabilitation
Excellent communication, documentation, and time management skills
Reliable transportation and valid driver's license for home visit travel
Compassionate, self-motivated, and comfortable working independently in a home setting
