About the Role:
Right at Home Des Moines is looking for a compassionate and skilled PRN Occupational Therapist to join our dedicated home care team in West Des Moines, IA. In this flexible per-diem role, you'll make a meaningful difference by helping clients regain independence and improve their quality of life — right in the comfort of their own homes. Responsibilities:

• Conduct comprehensive occupational therapy evaluations and assessments for home care clients
• Develop and implement individualized treatment plans to improve clients' daily living and functional skills
• Provide therapeutic interventions focused on ADLs, fine motor skills, cognitive rehabilitation, and home safety
• Recommend and train clients and caregivers on adaptive equipment and assistive devices
• Collaborate with interdisciplinary care teams including nurses, physical therapists, and care coordinators
• Document patient progress, treatment plans, and clinical notes in compliance with regulatory standards
• Educate clients and family members on home exercise programs and injury prevention strategies

Requirements:

• Graduate of an accredited Occupational Therapy program (Master's or Doctoral degree preferred)
• Active Iowa Occupational Therapist (OT) license in good standing
• NBCOT certification required
• Previous home health or home care experience preferred
• Strong knowledge of ADL training, adaptive equipment, and functional rehabilitation
• Excellent communication, documentation, and time management skills
• Reliable transportation and valid driver's license for home visit travel
• Compassionate, self-motivated, and comfortable working independently in a home setting
