About the Role:
Right at Home Des Moines is looking for a compassionate and skilled PRN Occupational Therapist to join our dedicated home care team in West Des Moines, IA. In this flexible per-diem role, you'll make a meaningful difference by helping clients regain independence and improve their quality of life — right in the comfort of their own homes. Responsibilities:
- Conduct comprehensive occupational therapy evaluations and assessments for home care clients
- Develop and implement individualized treatment plans to improve clients' daily living and functional skills
- Provide therapeutic interventions focused on ADLs, fine motor skills, cognitive rehabilitation, and home safety
- Recommend and train clients and caregivers on adaptive equipment and assistive devices
- Collaborate with interdisciplinary care teams including nurses, physical therapists, and care coordinators
- Document patient progress, treatment plans, and clinical notes in compliance with regulatory standards
- Educate clients and family members on home exercise programs and injury prevention strategies
Requirements:
- Graduate of an accredited Occupational Therapy program (Master's or Doctoral degree preferred)
- Active Iowa Occupational Therapist (OT) license in good standing
- NBCOT certification required
- Previous home health or home care experience preferred
- Strong knowledge of ADL training, adaptive equipment, and functional rehabilitation
- Excellent communication, documentation, and time management skills
- Reliable transportation and valid driver's license for home visit travel
- Compassionate, self-motivated, and comfortable working independently in a home setting
