Job Summary

The PFCC Coordinator is responsible for working with PFCC and clinical leadership to independently manage the system bed utilization process which includes Ben Taub, LBJ, and Hospital at Home campuses. The PFCC Coordinator collaborates with PFCC leadership, nursing and support services to ensure appropriate patient placement consistent with patient needs and ongoing hospital activities. The PFCC Coordinator monitors discharges and is responsible for notifying the PFCC Supervisor or designee when bed availability reaches critical levels or potential area-specific bottlenecks occur. In order to ensure efficient bed utilization and turnaround, this role is responsible for directing prioritization of bed cleaning in collaboration with EVS services. The PFCC Coordinator may also assist with coordinating medically-assisted transportation needs for patient discharges. The PFCC Coordinator complies with established standards as established by, national, state, and local regulatory agencies, and institutional policies and procedures. The PFCC Coordinator upholds high standards of integrity and ethical behavior, mentors others, role models professionalism, and participates in professional and community activities.

Minimum Qualifications

Degrees:

Bachelor's Degree Healthcare or Business Administration or any other healthcare-related field (Preferred)

School Education:

High School Diploma or GED

Licenses & Certifications:

Basic Life Support from approved American Heart Association (AHA) or American

Red Cross (ARC) Program Preferred

Licensed Vocational Nurse in the State of Texas Preferred

Work Experience:

Three (3) Years Work Experience in hospital bed management, patient placement, transfer center operations, or patient flow coordination within an acute care or multi-hospital health system preferred. Familiarity with Epic System preferred. Ability to

communicate effectively with interdisciplinary teams to support safe and appropriate patient placement in a fast-paced operational environment preferred.
