Job Summary

Review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability. Provide information and advice regarding drug interactions, side effects, dosage, and proper medication storage. he Home Infusion Pharmacy team provides safe, high-quality pharmacy services for patients receiving infusion therapies outside of the hospital setting. This role supports the preparation, verification, coordination, and monitoring of medications for home infusion patients while collaborating closely with providers, nurses, patients, and caregivers. Responsibilities may include medication order review, sterile compounding oversight, patient education, care coordination, inventory management, and ensuring compliance with regulatory and safety standards. Team members are expected to demonstrate strong communication, attention to detail, adaptability, and a commitment to delivering exceptional patient-centered care in a fast-paced healthcare environment.

Qualifications

In a retail/community pharmacy setting: Graduate of an accredited School of Pharmacy. Must have a thorough knowledge of drugs and their actions; knowledge of state and local regulations.

In a medical center or institutional setting: Graduate of an accredited School of Pharmacy. Must have a thorough knowledge of drugs and their actions; knowledge of state and local regulations. Hospital experience or residency preferred.

License must be in good standing by the State Board of Pharmacy in state(s) of practice and/or possess multi-state licensure privileges as required by position.
