We’re hiring a Registered Nurse (RN) – Admissions – a compassionate and skilled clinician who performs comprehensive evaluations, initiates care plans and ensures a smooth transition for patients entering hospice. This role is essential to delivering dignified, patient-centered care and supporting families during critical moments.

What You’ll Be Doing

  • Perform comprehensive nursing evaluations to admit patients into hospice
  • Assess for signs and symptoms of discomfort, including pain, and take prompt, appropriate action
  • Initiate care plans and provide appropriate nursing follow-up after hours
  • Instruct caregivers on managing patient needs and submit documentation within 24 hours of visit
  • Coordinate all services provided to patients and families across settings to ensure quality and appropriateness of care
  • Follow approved plans of care, modify as needed, and communicate with physicians when appropriate

Qualifications

  • Registered Nurse with a valid state license
  • Minimum of 1 year nursing experience required; hospice experience preferred
  • Ability to lift up to 50 pounds
  • Must clear criminal background check, physical, and drug screening
  • Valid driver’s license, good driving record, and reliable transportation required
  • Willingness to drive to patient locations and work outside regular hours as needed

Shift Schedule: PRN (Per Diem), Sunday and Monday, 12:00 PM - 8:00 PM

$53 - $58USD
