JOB SUMMARY

Coordinates all wound care into the total nursing care plan for patients, participates in patient and family teaching and provides leadership by working cooperatively with ancillary departments and other patient team personnel in maintaining standards for professional nursing proactive in the clinical setting.

JOB REQUIREMENTS

Minimum Education

  • Holds an active Licensed Practical Nurse license and is eligible to practice in the state of Kentucky. 
  • Current BLS certification.

Required Skills

  • Ability to work will with others.
  • Minimum Work Experience
  • Medical/surgical nursing experience with experience in chronic wound care preferred.

ESSENTIAL FUNCTIONS

  • Maintains the standards of nursing care and implements the policies and procedures of the hospital to patients on an inpatient and outpatient basis
  • Assesses the patient’s condition and nursing needs and performs appropriate nursing measures
  • Reports pertinent observations and reactions regarding patients to the appropriate person (i.e. physicians, supervisor etc.) records those observations accurately and concisely
  • Participates in team conferences
  • Administers medications and performs treatments to assigned patients within the department
  • Participates in economical utilization of supplies and ensures that equipment is maintained in a clean and safe manner.
  • Participates in orientation of personnel
  • Helps coordinate care of patients when scheduled for procedures with ancillary depts
  • Performs wound care treatments and documents appropriately – assist with care of patients on an inpatient and outpatient basis as needed
  • Assist with scheduling for wound care patients
  • Label prepare and locate files for nursing/physician documentation
  • Assist with digital photography, downloading and printing digital images
  • Responds to floating with a positive attitude
  • Collaborates with other healthcare team members to coordinate patient’s wound/ostomy care.
  • Performs and assist physicians in wound treatments of the patient.
  • Assist physician with procedures i.e. debridement, biopsy(s)
  • Observes, records and reports to the nursing supervisor, physician and other healthcare team members on patients condition and reaction to treatment, drugs and changes in condition.
  • Maintains the standards of nursing care and implements the policies and procedures of the hospital.
  • Assesses the patient’s condition and nursing needs; sets goals; prescribes appropriate nursing actions to meet those goals and the physical, psychological, social and rehabilitative needs of the patient, including discharge planning.

  • Provides education to patients/family regarding wound care and ostomy needs according to patient’s plan of care.
  • Implements and documents patient and family teaching as part of the plan of care
  • Accurately documents interventions in an appropriate and timely manner
  • Develops and documents discharge plan for outpatients with physician, patient/family and other available resources.
  • Serves as an educator, consultant/resource regarding wound, Ostomy, incontinent needs to hospital and medical staff.
  • Participates in the maintenance of a clean and safe environment.
