Arosa seeks a committed advocate to join our Adult Guardianship Team.

The candidate must be a motivated and reliable team player, flexible, organized, and detail-oriented, with excellent problem-solving and communication skills.

40 hours per week.

What is a Guardianship Care Manager

Guardianship Care Managers provide guardianship support and services in compliance with the North Carolina General Statute Chapter 35A.

This requires substitute decision-making, assessment, advocacy, monitoring, reporting, documentation and record maintenance, relationship building, liaison activities, and sometimes the ability to mediate challenging situations.

The Guardianship Care Manager will support clients 18 and older living with cognitive changes, MH/SA, or IDD, primarily in Wake County.

Responsibilities and Requirements

  • The position requires working independently and traveling throughout the region to visit individuals in their homes and other settings.
  • Monitor client health, safety, and well-being through regular visits (quarterly, at a minimum)
  • Provide crisis intervention, counseling, case management, and referral to programs as needed.
  • Serve as a surrogate decision-maker and advocate for medical, psychiatric, residential, vocational, and social needs.
  • Complete all company, state, and court-required documentation 
  • Review client charts and medical, mental health, and other service provider records to complete status reports, functional assessments, service plans, quarterly reviews, etc.
  • Apply for benefits, complete recertifications, and monitor trust accounts and expenses as needed. 
  • Participate in monthly team meetings, quarterly supervisions, and continuing education.
  • Candidate must have professional experience in the field of I/DD, mental health, and/or substance use.
  • A degree in a human service field is required.
