Program Coordinator – Adult Day Services (245D)

Location: Brainerd, MN
Schedule: Monday–Friday | 7:30 AM – 4:30 PM
No Nights • No Weekends • No Holidays

Salary: $40,000–$48,000 annually, depending on experience

Blue Sky Support Services is looking for a compassionate, organized leader to join our Adult Day Services team as a Program Coordinator. In this role, you'll help adults with disabilities achieve their personal goals while supporting a team of Direct Support Professionals and ensuring high-quality, person-centered services.

This position serves as a Minnesota 245D Designated Coordinator and plays a key leadership role in coordinating services, supporting staff, and maintaining compliance with state licensing requirements.

What You'll Do

  • Coordinate and oversee person-centered services for program participants.
  • Partner with participants, families, guardians, case managers, and interdisciplinary teams.
  • Lead support team meetings and monitor progress toward individual goals.
  • Ensure services are delivered according to each person's Coordinated Service and Support Plan (CSSP) and related support plans.
  • Train, mentor, and support Direct Support Professionals to provide exceptional care.
  • Monitor documentation for accuracy, completeness, and regulatory compliance.
  • Assist with participant assessments, program development, and quality improvement initiatives.
  • Support a positive, inclusive, and respectful team culture.
  • Perform other related duties as assigned.

Minimum Qualifications - Applicants must meet Minnesota 245D Designated Coordinator qualifications, including one of the following:

  • Bachelor's degree in a human services-related field and one year of full-time direct care experience.
  • Associate degree in a human services-related field and two years of full-time direct care experience.
  • Diploma in a human services-related field and three years of full-time direct care experience.

OR

  • At least 50 hours of education and training related to human services and disabilities; and
  • Four years of full-time direct care experience providing services to individuals with disabilities or adults age 65 or older under qualified supervision.

Additional Requirements

  • Must successfully pass a Minnesota DHS background study.
  • Valid Minnesota driver's license with an acceptable driving record.
  • Reliable, insured personal transportation.
  • Ability to respond professionally to work-related calls, texts, and emails as needed.
  • Ability to lift, assist, and participate in activities as necessary to support participants.
