Provides support to Day Habilitation Services through the provision of training education and assists individuals to realize their potentials through a variety of activities that include social skills acquisition community connections engagement in meaningful activities and vocational training and employment.Essential DutiesEssential Duties and Responsibilities including but not limited to the following as well as any other duties as assigned by supervisory staff.

Daily monitoring and completion of computerized program tasks including but not limited to Dashboard Progress Notes Daily Training Record Notes Time America and Case Workload.

Responsible for overall management of day to day operations of Adult Day program.

Ensures quality programming and services through the development and maintenance of appropriate program services.

Facilitates program growth and development according to identified needs. Represents and promotes program services within the community through speaking engagements.

Coordinates admission process for all new referrals to the adult day program.

Provides oversight and ensures proper facilitation and documentation of service planning meetings and interdisciplinary team meetings.

Implements programming with the structure and intensity necessary to meet the needs of the individuals engaged in our treatment through creativity and therapeutic groups.

Organizes all approved outings and identifies its purpose and therapeutic value.

Reviews Medication Administration Record binder and CDS Sheet to ensure all forms are completed accurately ensure adequate supply of medications are present at location.

Coordination with outside services family members and treatment providers to enhance the continuum of care. To be able to provide individuals with proper referral when needed including but not limited to connecting member with an outside psychiatrist division of vocational services social services etc.

Review and completion of incident reports to ensure compliance with reporting requirements.

Oversee the development and maintenance of individuals files including chart audits to ensure files are maintained in compliance with all regulatory standards.

Facilitates weekly treatment team meeting and monthly community meeting for adult day program.

Completes monthly facility inspections and rectifies or reports any concerns to ensure the safety of all individuals at the adult day program location.

Will oversee documentation and have the ability to clearly and thoroughly document observations of individual behaviors and responses to specific interventions.

Ability to assess an individual&rsquos status including progress towards goals as well as identifying areas that need improvement.

Responsible for coordination of individuals to and from day program with the Transportation Manager Drivers and Outside Providers.

Ensures that the behavioral and pharmacologic approaches to treatment are evidence based or based on objective information to provide treatment services consistent with recognized treatment principles and practices for each level of care and type of client served by the program.

Delegates the completion of Fire and Disaster Drills ensure the completion and accurate documentation of all Drills.

Conducts trainings program for professional development including medical needs of individuals behavioral interventions Psychadvantage and other in house systems.

Participates in appraising qualifications of candidates for open positions.

Completion of monthly supervision performance evaluation and progressive discipline as needed for staff.

Participates in group evaluations quarterly and provides observational feedback during supervision.

Ensures that all groups are adequately staffed and adheres to the staff to member ratio set forth by Program Standards.QualificationsTo perform this job successfully an individual must be able to perform each essential duty satisfactorily. Must be 18 years or older and possess a valid non-probational driver&rsquos license for Supported Employment staff must be 20 years or older and posses a valid non--probation drivers license.

  • Clearance from the Department of Human Services following a federal and state criminal background check as well as checked against the Central Registry and Child Abuse Record Information.
  • Pre-employment physical Pre-employment tuberculosis test Pre-employment and random drug screen as required by The State of New Jersey Department of Human Services.
  • Clearance based on a 3-year and annual motor vehicle abstract assessment.
  • Must be able to successfully complete required training which includes but is not limited to

American Red Cross Adult CPR and First Aid

Crisis Management

Medication Administration with on-site competency assessment

Preventing Abuse and Neglect with on-site competency assessment

Positive Behavior Supports

Employment Specialist Foundations if applicable

12 hours of professional development annually per calendar year prorated at one hour per month
