We are currently seeking to fill two Program Coordinator positions for different programs: SHOUT and SMI

A. Program Coordinator-SHOUT

The Program Coordinator will assist the Project Director for the SHOUT (Street HIV Outreach to Engage Unsheltered Populations in Treatment) program with program implementation and the day-to-day operations of the program. The Program Coordinator will provide outreach services in streets, soup kitchens, shelters, and encampments. The Program Coordinator will work collaboratively with members of the Greater New Haven Healthcare for the Homeless department to ensure the program meets contractual requirements, including data management related to client enrollment, demographics, services provided, and program administration costs. Direct service responsibilities may include identifying individuals at risk for HIV, HCV, and STIs; conducting screenings and testing; providing community education and outreach; assisting patients in obtaining STI treatment and prevention services; delivering harm-reduction services; and linking patients to mental health care and substance use disorder treatment programs.

This is a full-time position requiring 80% travel throughout the greater New Haven area. The Qualified person will be comfortable with working outside in all weather conditions with those experiencing homelessness in the greater New Haven area.

Key Responsibilities:

  • Conducts street canvassing and outreach within the New Haven Region to offer unsheltered individuals screenings, prevention, and treatment for STI's including HIV and HCV, and linkages to other services including mental health and substance use disorder treatment.
  • Assists providers in maintaining registry of unsheltered patients receiving treatments.
  • Ensure GPRA data is entered timely and accurately for grant monitoring.
  • Will perform outreach during non-traditional hours to include early late and weekend hours as needed.
  • Must be willing and be flexible to work at least two early mornings and two evening shifts per week
  • Works as a team in support of the medical providers when assigned.
  • Responsible for ensuring notes are entered in Epic, Homeless Management Information System (HMIS), And DDAP systems per program requirement and data quality standards.
  • Operates and maintains the mobile office ensuring clean, safe operation.
  • Follows-up with referral sources in a timely fashion as required.
  • Attends inter-agency meetings when directed, participating, and fostering collaborative relationships.
  • Completes applications for Basic Needs, transportation, and treatment programs in a timely manner.
  • Coordinates with community agencies for outreach/engagement. Developing strong working relationships with community partners is of the essence.
  • Those with medical qualifications will be expected to use them to the level of their training to assist providers in the field.
  • Performs all other duties as assigned by the program director, or department managers.

B. Program Coordinator-SMI

The Program Coordinator for Street Medicine Interventions supports the implementation, coordination, and evaluation of Cornell Scott-Hill Health Corporation's Street Medicine Demonstration Project focused on unsheltered individuals living with HIV. This position plays a central role in coordinating street-based outreach activities, supporting integrated HIV, primary care, behavioral health, and social service delivery, and ensuring accurate documentation, data collection, and reporting required by the grant. The Program Coordinator SMI works collaboratively with Infectious Disease (ID), Street Medicine and external grant-funded partners to help adapt, document, evaluate, and disseminate evidence-based street medicine interventions.

This is a field-based position requiring extensive travel throughout the Greater New Haven area and regular engagement with unsheltered individuals in non-traditional care settings. It requires flexibility to work non-traditional hours, including early mornings, evenings, nights, and occasional weekends, in order to meet the operational and patient care needs of the Street Medicine Project.

Key Responsibilities:

  • Coordinate and support street-based outreach activities targeting unsheltered individuals living with or at risk for HIV in the Greater New Haven area, including streets, encampments, drop-in centers, and other non-traditional care settings.
  • Support integrated service delivery models that include HIV treatment and prevention, primary care, STI screening and treatment, behavioral health services, substance use disorder services, and social support.
  • Assist clinical and outreach staff with scheduling, follow-up, and continuity of care for participants engaged through the demonstration project.
  • Promote trauma-informed, overdose prevention, and patient-centered approaches when engaging in unsheltered individuals.
  • Ensure accurate and timely data collection related to outreach encounters, services delivered, outcomes, and participant engagement in accordance with grant requirements.
  • Enter and maintain documentation in required systems including Epic,HMIS, REDCap and other grant-mandated data platforms.
  • Support monitoring of project performance measures, benchmarks, and participant enrollment targets (including annual service targets).
  • Assist with preparation of reports, data summaries, and documentation for internal review, funders, and evaluation partners.
  • Collaborate with the Evaluation Provider to support implementation-science activities, including data collection, fidelity monitoring, and evaluation processes.
  • Those with medical qualifications will be expected to use them to the level of their training to assist providers in the field.
  • Performs all other duties as assigned by the program director, or department managers

Program Coordinator Requirements:

  • High school diploma/GED required. Associate degree or bachelor's degree preferred.
  • CPR Certification
  • MA or CNA, or EMT training preferred.
  • Human Service experience is preferred.
  • Excellent working knowledge of New Haven community social and health service agencies is preferred.
  • Excellent communication skills; both written and verbal.
  • Bilingual ability preferred.
