Responsibilities

• Coordinate day-to-day implementation of Rural Health Transformation Program activities across participating counties and partner organizations.
• Develop and monitor project workplans, implementation timelines, milestones, and deliverables.
• Ensure alignment of activities with contractual requirements, funding agency expectations, and program objectives.
• Monitor risks, barriers, and implementation challenges and develop mitigation strategies.
• Facilitate communication among project leadership, implementation teams, healthcare partners, and community stakeholders.
• Interpret and apply university, sponsor, state, and federal policies to ensure compliance with programmatic and funding requirements.
• Develop and maintain relationships with healthcare organizations, community-based organizations, public health partners, local government leaders, and regional stakeholders.
• Plan and facilitate advisory committee meetings, learning collaboratives, and stakeholder engagement activities.
• Coordinate communication strategies to promote collaboration across participating organizations.
• Support dissemination of best practices and lessons learned.
• Monitor implementation and performance metrics across all project components.
• Work closely with the Data Manager to review program outcomes, identify improvement opportunities, and support data-informed decision making.
• Coordinate quality improvement initiatives with participating healthcare and community partners.
• Support development and implementation of performance improvement action plans.
• Monitor project budgets and expenditures.
• Coordinate contracts, subawards, consultants, and vendor agreements.
• Track utilization of project resources and ensure compliance with funding requirements.
• Assist project leadership with budget forecasting and resource allocation.
• Coordinate preparation of progress reports for funding agencies and stakeholders.
• Assist in development of presentations, summaries, policy briefs, and program reports.
• Support dissemination of project outcomes, implementation successes, and lessons learned at state and national forums.

Qualifications

• Bachelors degree and four years of experience related to health and human services; or a high school diploma/equivalent and experience equal to eight years. (Note: post-high school education can substitute for experience at the equivalent rate.)
• The incumbent must have reliable transportation, and a valid Florida or Georgia Drivers License or the ability to obtain prior to hire.

This position will be supporting Bundle 1 - Northwest region, which is defined as Bay, Calhoun, Escambia, Franklin, Gadsden, Gulf, Holmes, Jackson, Jefferson, Leon, Liberty, Okaloosa, Santa Rosa, Wakulla, Walton, and Washington counties on the Rural Health Transformation Program Map.

This position requires regular travel in support of program activities and responsibilities. Travel may occur within the employee's assigned region and throughout the State of Florida, as needed. The frequency and location of travel may vary based on programmatic and operational needs.
