Your Role
The Medi-Cal Operational Readiness team leads cross-functional implementation and operational readiness for Medi-Cal programs, including CalAIM initiatives that support members experiencing homelessness. The Program Manager, Senior - Coordinated Entry System will report to the Senior Manager, Program and Project Management. In this role, you will serve as Blue Shield's primary connection to local homelessness response systems, community-based organizations, and Continuums of Care. You will help ensure Blue Shield's investments in street medicine, Enhanced Care Management, Community Supports, Transitional Rent, and related homelessness programs are aligned toward housing stability for members while strengthening program effectiveness, accountability, and real-world outcomes.

Your Knowledge and Experience
- Requires a bachelor's degree or equivalent experience
- Requires at least 5 years of prior relevant experience
- Required experience working with homelessness response systems, Coordinated Entry Systems, Continuums of Care, community-based organizations, or housing and social service partners
- Requires experience managing cross-functional programs, operational workflows, stakeholder engagement, issue resolution, and implementation activities
- Requires ability to translate community, provider, and member needs into operational recommendations, process improvements, and actionable program insights
- Knowledge of Medi-Cal, CalAIM, Enhanced Care Management, Community Supports, Transitional Rent, street medicine, redetermination, or managed care operations strongly preferred
- Experience using data, dashboards, HMIS information, or outcomes reporting to monitor program performance and support accountability strongly preferred
Hybrid
This role requires employees to be in-office based on our hybrid workplace model, balancing purposeful in-person collaboration with flexibility. For most teams, this means coming into the office two days each week.
Employees living more than 50 miles from an office location will work with their manager to determine in-office time based on business need.
Your Work
In this role, you will:
- Serve as Blue Shield's primary liaison to Coordinated Entry System partners, Continuums of Care, community-based organizations, and regional homelessness response systems
- Represent Blue Shield in CoC board meetings, workgroups, regional planning meetings, and other community forums, including LAHSA and RTFH engagement
- Provide training and technical assistance to CES partners and community-based organizations on Medi-Cal benefits available to members experiencing homelessness, including ECM, Community Supports, non-emergency medical transportation, pharmacy benefits, street medicine, Transitional Rent, and redetermination
- Serve as the designated escalation point for CES partners and community-based organizations when support is needed for Blue Shield members experiencing homelessness
- Support end-to-end implementation of homelessness-related CalAIM initiatives, including Transitional Rent, Enhanced Care Management, Community Supports, and related operational workflows
- Lead community intelligence gathering by identifying local priorities, system barriers, emerging opportunities, and operational gaps that affect members experiencing homelessness
- Support the Street Medicine Dispatch Program by managing incoming internal requests, coordinating with street medicine providers, and tracking outcomes and follow-up activities
- Track and report housing-related outcomes, including members engaged in CES, members connected to services, members successfully housed, and Transitional Rent members reconciled in HMIS
- Conduct field engagement and on-site visits with community partners to monitor homelessness-focused investments, including HHIP initiatives, street medicine programs, and other community-based efforts
- Maintain internal homelessness resources, guidance, and community contact information to support cross-functional teams and improve coordination across programs
